Assessment

In order to ensure an adequate and recovery/resiliency focused assessment, Comcare agency uses a variety of methods to gather assessment data regarding a recipient. This data is collected in the form of an assessment document that is completed. This data is the foundation of the recipient’s Individualized Treatment Plan.

 

Professional Treatment Plan

The Treatment Plan is an individualized, structured, goal-oriented schedule of services developed jointly by the recipient and treatment team. The Treatment Plan identifies the goals, objectives, interventions, and units of service which are based on the results of an Assessment and agreed to by the adult or youth and his/her parent/guardian.

 

Skills Training and Development

Skill Training are goal-directed supports and solution-focused interventions intended to achieve identified goals or objectives as set forth in the individual’s individualized treatment plan. The service provides opportunities for the client to acquire and improve skills needed to function as appropriately and independently as possible in community.

Counseling and Therapy

Outpatient treatment is typically individual, family and/or group outpatient psychotherapy, consultative services, mental health assessment, evaluation and testing. Times for provision of these service episodes range from fifteen minutes (e.g., medication checks) to fifty minutes (e.g., individual, conjoint, family psychotherapy), and may last up to two hours (e.g., group psychotherapy).

 

Crisis intervention service

This agency will maintain a 24-hour crisis response system to provide services to the agency’s recipients during period of emergency needs. This system will includes capacity for face-to-face services and the availability of the LMHP and Psychiatrist on an emergency basis.

 

Psychosocial rehabilitation (PSR)

PSR services are designed to assist the adult with compensating for or eliminating functional deficits and interpersonal and/or environmental barriers associated with their mental illness. The intent of PSR is to restore the fullest possible integration of the individual as an active and productive member of his or her family, community and/or culture with the least amount of ongoing professional intervention.

 

Medication Management

Comcare support center provides pharmacotherapy services that include evaluating and prescribing medications to recipients in response to specific symptoms, behaviors and conditions for which the use of medications is indicated and efficacious.
